The Health Assessment and Planning Partnership quarterly meetings are designed to:
- Foster networking opportunities
- Provide feedback on the community health assessment and planning process
- Hear updates on the Community Health Needs Assessment (CHNA) and Community Health Improvement Plan (CHIP)

Hello all – This HAPP meeting will be dedicated to prioritizing the community health issues for the next Community Health Needs Assessment (CHNA). You will have an opportunity to “vote” on the 30+ indicators and contribute (along with other groups) to identifying the top health issues in our community.
Also, for those who haven’t seen this already, attached below is the announcement for the “Mayo Clinic Shared Value Award.” As a core partner in the CHNA process (along with Olmsted Medical Center and Olmsted County Public Health ), this award is an opportunity to further strengthen excellent work in the community that aligns with the CHNA priorities.
